Health professionals appointed for 2010-2011

Northern Lehigh School Board last Monday appointed Dr. Nicole Sully and Dr. Amy Steigerwalt of Parkland Family Health Center as school physicians for the 2010-11 school year.

The fee for grade level physicals will be $20 per physical needed. Consultant services will be provided at no charge.Dr. Harry Shleifer of Professional Dental Associates was appointed as the school dentist for the 2010-11 school year at the quoted fee of $1 per dental exam needed.OAA Orthopedic Specialists was appointed to provide football game physicians for the 2010-11 school year at a quoted fee of $75 per regular season game and $85 per post-season game. This is no increase over last year's fee.The board also appointed Dr. Harry Shleifer of Professional Dental Associates as the school dentist for the 2010-11 school year at the quoted fee of $1 per dental exam needed.In other business, the board:• Awarded insurance coverage for the 2010-11 school year to: Commercial Package and Automobile (Vehicle Liability, General Liability Propety Insurance and Boiler and Machinery) to Graphic Arts (UTICA) National Insurance Company at a cost of $58,451, which represents a decrease of $1,193 over last year's rate; Commercial Umbrella Liability Insurance - North River Insurance Company - at a cost of $15,810, which represents a $306 increase over last year's rate; School Leaders Error and Omissions Liability - National Union Fire Insurance Company - $12,569, which represents no increase over last year's rates; and Workers' Compensation Coverage - School Boards Insurance Company - at an estimated premium of $56,495, which represents about a $10,851 decrease. This amount is an estimated total, with the final cost based on actual payroll figures.• Approved the appointment of Statewide Tax Recovery as the delinquent per capita tax collector for the 2010-11 school year.• Approved Remco, Inc, as a preventative maintenance service provider for the cafeteria equipment in all four buildings at a cost of $3,966, which represents no increase over last year.• Appointed Mid-Penn Insurance Associates as the 2010-11 Student Accident and Athletic Insurance Carriers. The renewal rate is $7,595, an increase of $691 over last year, to include all interscholastic sports, band and cheerleading. Voluntary school coverage is $22 for the school time rate and $88 for the 24-hour rate, which represents no increase in the rate from 2009-10.• Approved to renew the expiring $50,000 Board Secretary Bond at an annual premium of $175, as well as a $50,000 Board Treasurer Bond at an annual premium cost of $269, for the term of July 1, 2010 to July 1, 2011, for the 2010-11 fiscal year through HRH Pally Simon Associates.• Appointed Hilb, Rogal and Hamilton Company (HRH) Palley Simon Associates as the district insurance broker for Property, Casualty, Liability and Worker's Compensation insurance for the 2010-11 school year.• Authorized administration to lease data projectors for the middle school, as well as laptops and carts, interactive white boards and data projectors for Peters Elementary, at a cost of $48,000 per year, with a lease term of 36 months and a $1 buyout option at the end of the lease agreement, from from California First Leasing Corporation, Irvine, CA, 92612.• Authorized administration to contract with Miller Flooring Company, Inc., to repair, sand and refinish the high school gymnasium floor at a cost of $33,670 (State Contract Price), to be paid from the Long Range Maintenance Plan Budget.• Authorized administration to purchase a new Convection, Electric Floor Model Steamer for the middle school cafeteria at a cost of $15,073 (State Contract Price) from Singer Equipment Co., to be paid from the Cafeteria Fund.• Authorized administration to purchase a new Mobile Heated Cabinet for the Peters Elementary School cafeteria at a cost of $4,666 (State Contract Price), from Singer Equipment Co., to be paid from the Cafeteria Fund.• Approved the 2010-14 Computer Technicians Agreement between the Northern Lehigh School District and the Northern Lehigh Computer Technician.• Adopted the Carbon Lehigh Intermediate Unit's Notice of Adoption of Policies, Procedures and Use of Funds by School District for submission with the 2010-11 IDEA B, Section 611 project application, as presented.• Authorized proper officials to execute contracts for Title I and Title IIA for the 2010-11 school year.- Approved the daily tuition rate of $52.55 for the PATH PA Treatment & Healing Center, Inc.• Approved a maintenance agreement for the 2010-11 school year with Allentown Sewing Machine Outlet for a total rate of $430.• Authorized the financial institutions of First Niagra, Wachovia National Bank, Commerce Bank of Pennsylvania, Prudential-Bache Government Securities Trust, and Pennsylvania Local Government Investment Trust (PLGIT) to act as approved depositories for school district funds and/or temporary investments.• Approved a request by Karen Nicholas to attend the Standars Aligned System Integration Trainer conference from June 13-15, 2010, at the King of Prussia, PA. Cost for travel is $52, and meals, $50, for a total approximate cost of $102, to be funded from the Curriculum and Instruction budget.• Approved a request by Susan Bahnick to attend the Orientation for New and Aspiring School Foodservice Directors conference from Aug. 2-4, 2010, at State College, Pa. Cost for travel is $167, meals, $120, and lodging, $228, for a total approximate cost of $515, to be funded by a grant.• Approved a request by Mary Kay Williamson to attend the National Autism conference from Aug. 1-6 at State College, Pa. Cost for registration is $150, travel, $170, meals, $200, and lodging, $515, for a total approximate cost of $1,035, to be funded from the Special Education Department.• Approved the request of Shawn Green to take an unpaid day on May 28, 2010.• Approved the request of Jillian Mlynek, to take 3 unpaid days Oct. 6, 7, and 8, 2010.
